Essential Duties and Responsibilities
- Deliver therapeutic interventions designed by a supervising Physical Therapist.
- Provide strengthening, ROM, balance, and mobility training in the home setting.
- Track improvements, document visits, and communicate changes using an intuitive EMR.
- Teach exercises, safety techniques, and functional strategies to patients and caregivers.
- Partner with PTs, nurses, and other clinicians to ensure coordinated, high-quality care.
Knowledge, Skills and Abilities
- Reliable, insured vehicle and valid driver's license.
- Strong communication skills and a patient-centered mindset.
Education & Experience
- Home care experience preferred, but comprehensive onboarding is provided.
