Job Description
A recognized health services organization is actively seeking a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to provide clinic services to youth in a school-based setting.
About the Opportunity:
- Schedule: Monday to Friday
- Hours: 8am to 4:30pm
- Setting: School-based Mental Health Clinic
Responsibilities:
- Maintain an average clinic caseload of approximately 25-30 students grades kindergarten - 12th grade
- Participate in all admissions with the child, family, and telehealth MD through
- Work collaboratively with our telehealth fellows on each youth's treatment needs
- Provide Individual and Family Therapy and crisis intervention as needed
- Advocate for youth and families to obtain community-based linkages with additional supportive services that meet their needs
- Maintain timely medical record documentation
- Facilitate thorough discharge planning and complete discharge follow-up
- Perform other duties, as needed
Qualifications:
- Bachelor's and/or Master's Degree
- Active NYS Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW)
- Prior experience working with Adolescents / Children with Mental Health diagnoses
- Prior experience working in a School-based setting
